Link Worker - Ways to Wellness - Full Time Contract Position

Link Worker Ways to Wellness – Contract Role

Mt Gravatt Community Centre is a not-for-profit, registered charitable organisation located on Logan Road, Upper Mount Gravatt. Our centre serves as a hub for the community, offering a diverse range of services and activities accessible to everyone.

Ways to Wellness Project: The Ways to Wellness Project is an initiative funded by the Queensland Department of Treaty, Aboriginal and Torres Strait Islander Partnerships, Communities and the Arts and is also supported by the Queensland Community Alliance. It focuses on Social Prescribing—a community network approach where socially isolated and/or lonely individuals are referred to a Link Worker who supports them to engage with meaningful community groups, activities, hobbies, services and supports to enhance their sense of wellbeing and belonging.

The Position: The Link Worker will play a crucial role in integrating referred individuals into their local communities to improve their health and well-being through social connection and inclusion. The role also involves working closely with GPs, Primary Care Teams, Allied Health providers, and other organisations to enhance referral pathways and community cohesion and embed evidence based and Social Prescription practices.

Key Responsibilities:

Client Services: 

·         Provide client-centred support through strengths-based intake and assessment and case management, utilising professional expertise.

·         Assist clients in achieving personal health and wellbeing goals by connecting them with community-based groups, activities, services and supports.

·         Apply evidence-based practices that effectively address client needs including using trauma informed, strengths based, person centred frameworks

·         Manage comprehensive client assessments and case management, ensuring all interactions are planned and structured to improve health, connection, and well-being outcomes.

·         Maintain accurate and comprehensive records, including reports for various stakeholders.

Community and Strategic Engagement: 

·         Engage actively with the community, promoting Social Prescribing and fostering social inclusion and belonging.

·         Develop and maintain relationships with internal and external stakeholders, including medical professionals, government and non-government agencies, and community organisations.

·         Identify and implement continuous quality improvement opportunities aligned with MGCCI values.

·         Participate in mandatory training and professional development programs as required.

Selection Criteria:

·         Tertiary Degree in Social Work, Psychology, Human Services, Public Health/Health Promotion, or a related field.

·         Minimum of two years of relevant community work experience.

·         Strong understanding of evidence-based frameworks including person-centred and trauma- informed approaches.

·         Demonstrated ability to work with disadvantaged and vulnerable individuals or those facing significant barriers.

·         Excellent clinical reasoning, problem-solving, and communication skills.

·         Experience in developing and maintaining relationships with various stakeholders.

·         Ability to work autonomously while contributing to a collaborative team environment.

Proficient in using the Windows Operating System and Microsoft Office applications (Word, Excel, PowerPoint).

·         Flexibility to work occasional evenings to meet service needs.

·         Valid Driver’s License, Police Check, and current Blue Card required.

Desirable Skills:

·         Knowledge of local agencies and services in Brisbane’s Southside.

·         Familiarity with social prescribing.

·         Experience in managing databases.

·         Marketing experience related to service promotion (not required but advantageous)

About the Program: Ready for Work is part of the Queensland Government’s Skilling Queenslanders for Work initiative, designed to help job seekers transition into the workforce by providing them with essential practical skills. The program is particularly focused on supporting individuals who are ineligible for Australian Government employment services or who are disadvantaged and require additional assistance.

The Role: As the Ready for Work Program Facilitator, you will play a key role in delivering this impactful program, guiding participants through a structured course designed to enhance their employability. This role is ideal for someone who is passionate about helping others achieve their goals, particularly individuals facing barriers to employment.

Key Responsibilities:

·        Facilitate Training: Deliver job search assistance and training, including resume preparation, interview skills, job search strategies, and understanding employer expectations.

·        Support Participants: Provide tailored guidance to participants, helping them to develop the skills and confidence needed to enter the workforce.

·        Networking: Foster relationships with local businesses and employers, facilitating opportunities for participants to network and learn from industry professionals.

·        Documentation: Maintain accurate records of participant progress and program outcomes, ensuring all required reports are completed and submitted to stakeholders.

·         Community Engagement: Actively promote the program within the community, engaging with potential participants, businesses, and other stakeholders.

Selection Criteria:

  • Relevant experience in training, career coaching, or employment and/or community services.
  • Strong understanding of the challenges faced by disadvantaged job seekers and a commitment to providing supportive, inclusive services.
  • Excellent communication and interpersonal skills, with the ability to engage effectively with participants, businesses, and community organisations.
  • Experience in facilitating group sessions or workshops is highly desirable.
  • Ability to work autonomously while contributing to a collaborative team environment.
  • Proficient in using the Windows Operating System and Microsoft Office applications (Word, Excel, PowerPoint).
  • Flexibility to work occasional evenings to meet service needs.
  • Valid Driver’s License, Police Check, and current Blue Card required.

Desirable Skills:

  • Knowledge of local job markets and employment services in Brisbane’s Southside.
  • Experience in community outreach or working with diverse populations.
